FREQUENTLY ASKED QUESTIONS

DO WE NEED TO BOOK A FACE TO FACE APPOINTMENT OR A TELEPHONE APPOINTMENT?

Both options are available. 

If a patient has had all their measurements taken, a telephone appointment may be suitable. 

If all measurements need to be taken, the a face to face appointment is required.

WHERE DO YOU SOURCE ELIGIBLE PATIENTS LIST FROM?

  1. log into practice EMIS 

  2. Click population reporting

  3. Select "One Health Lewisham Ltd" within the Search & Report section (bottom left). You may need to pull tab up to view. 

  4. Select OHL NHS Healthcheck folder > face to face appointment, or > telephone appointment folder

  5. Population included = the patient to book into the relevant slots. 

WHICH EMIS TEMPLATE SHOULD I USE?

Ardens v16.5. Please ensure you have uploaded this template and the protocols provided by OHL. Any questions, please do not hesitate to get in touch with us.

WHAT HAPPENS IF A PATIENTS HAS NOT HAD THEIR BLOOD TEST?

The HCA at the OHL Hub will take their bloods at the appointment. You will need to code the results when they are received at the practice.

WE DO OUR OWN HEALTHCHECKS - WILL THIS BE COUNTED TOWARDS THE PHS TARGET?

Yes

HOW DO WE KNOW WHAT OUR HEALTH CHECK TARGET IS?

As part of the PHS, One Health Lewisham issues a monthly dashboard to practices, to help with tracking and monitoring progress.
