PCN Physician Associate

Salary: From £37,000, depending on experience

Hours: 37.5 p/w – Monday-Friday

The role of Physician Associate will be for the GP practices that form the North Lewisham Primary Care Network (NLPCN). The NLPCN is one of the largest PCNs in Lewisham and has a very diverse population made up of 10 GP Practices.

Health inequality impacts their patients: North Lewisham has higher than average levels of deprivation and BAME populations and poorer health outcomes.

NLPCN is committed to tackling health inequality as a priority and have developed a program of work to improve trust and engage with the local community, training GP surgeries and staff to improve access for patients, improving our communication to patients about service changes and health promotion and improving the monitoring of those suffering from health inequality to drive our strategy.

The Physician Associate will work closely with the clinical lead, GP/practice nurses and the ANPs or other advanced practitioners.

Job responsibilities include:

  • Provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems, utilising history-taking, physical examinations, and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient
  • Provide clinical support for:
    • PCN outreach, health checks and specialist clinics (e.g. chronic disease clinics, women’s health clinic, diabetes, learning disabilities, COVID vaccinations)
    • care homes, home visits… as directed/required by the clinical lead/directors
  • Undertake face to face, telephone and online consultations for emergency or routine problems, as determined by the Practice, including management of patients with long-term conditions. Participate in all treatment and preventative healthcare services in the PCN Practices as delegated and agreed by the clinical lead/mentoring GP
  • Assist care coordinators with clinical issues relating to urgent care plans and coordinating care for patients and communicate, when necessary, with colleagues in Primary Care and hospital specialists in order to discuss or refer specific patients
  • Order and interpret investigations, conduct post discharge reviews, new admission reviews, acute assessment, care plans, CGAs and assist with Improved Access across the PCN
  • Give clinical instructions to nursing staff and primary care team as required
  • Instruct and educate patients in preventative health care
