It builds on the work that has been done by the  Darzi  and HEE GP Fellow and Time for Care programme including :

  • Ensure we deliver on the outputs from Time for care, ensuring the registrations processes in place, and searches to facilitate access for those who may be excluded by the digital first approach
  • Building the links with local 3rd sector organisations  and GP Practices and PCN staff, including utilising local influencers to support preventative health care and early health seeking behaviour for significant symptoms including cancer
  • Frequent Attenders in General Practice with the Darzi Fellow Programme embedding continuity of care, care planning and system change

There will be support both from mentoring within the fellowship programme and from with the PCN with clinical supervision and staff to  work with you to support delivery

It will be funded at £9,500 / portfolio session. There needs to be agreement with your employing practice as to whether they reduce your commitment so it is done within existing working hours or if you do an additional session as a fellow.
