Vacancies within Sevenfields Primary Care Network (PCN)

Sevenfields PCN, a group of six varied GP practices in Downham, Grove Park and Catford is working together to improve the health of our local residents, in particular those at risk of or who already have diabetes. To this end, we are seeking to recruit a team of all the talents: Physician Associates, Nursing Associates, Pharmacists, Health Coach and Dietician to work in or across our GP practices and with our specialist Diabetes Team.

Sevenfields Primary Care Network (PCN) is one of the largest PCNs (of six) in Lewisham, and is seeking to be a key driving force for improving community health and wellbeing in our neighbourhood. Our PCN is made up of 6 GP Practices serving a population of around 65,000 patients. We are led by a local GP who performs the role of Clinical Director: Dr Helen Tattersfield (Oakview Family Practice).

Sevenfields PCN – Community Dietician

Full time – 37.5 hours

Salary – £40-43k Depending on Experience

We are now recruiting a Community Dietician to join our team. This is a brand-new role and a rare opportunity for a dietitian to develop effective and successful models of dietetic care in diabetes management within primary care, enhancing existing pathways of care while advancing their own practice and developing as a first contact practitioner

Main Duties and Responsibilities include:

• Work as part of a multi-disciplinary team in a patient facing role to assess and treat patients using expert knowledge and completion of individual care plans.
• Support the patients of Sevenfields PCN with dietary interventions primarily but not exclusively in the management and prevention of diabetes.
• Coach and motivate patients through multiple sessions, to identify their needs, set goals and support them to implement their personalised health and care plan, in individual and, where appropriate, group sessions, either remotely or face-to-face.
• Assess, plan, implement and evaluate therapy intervention to meet the assessed needs of patients.
• Complete a range of written reports, as appropriate.
• Actively signpost patients to the correct healthcare professional.
• Maintain accurate clinical records of all patient consultations and related work.

The post holder will be employed by individual practices working alongside the practice team in all areas of their work but with a special interest in diabetes. They will work with our Health Coach to provide support to patients wishing to make positive lifestyle changes for all our practices. There will be opportunities for training and development and participation in many exciting and innovative schemes within the PCN.

The role is offered full-time, 37.5 hours per week. Applicants will be expected to have qualifications appropriate to the position they are applying for and be comfortable with supporting a positive lifestyle and diet approach to the management of diabetes.

This role may involve working with specific grounds (i.e. children and young people) depending on the applicants experience. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Sevenfields PCN – Community Health Coach

Full time – 37.5 hours

Salary – £25-28k Depending on Experience

We are now recruiting a Community Health Coach to join our team. The post holder will have a key role in supporting our innovative Diabetes Prevention and Reversal Lifestyle Project – as well as supporting the GP Practices that make up our PCN. They will also work closely with the Neighbourhood Development Programme and will support service integration work around mental health, social care, community nursing and the voluntary sector.

Main Duties and Responsibilities include:

  • To be responsible for the assessment of new patients with subsequent production and completion of individual care plans.
  • To provide personalised support to individuals, their families and carers, to ensure that they are active participants in their own healthcare, and to empower them to take more control in manging their own health and well-being, to live independently and to improve their health outcomes
  • To work with the social prescribing service to connect them to community-based activities which support their health and well-being.
  • To be aware of limitations and, as required, be responsible for the referral of patients who require the intervention of other healthcare professionals.
  • To provide support to local community groups and work with other health, social care and voluntary sector providers to support patients’ health and well-being holistically.
  • To actively signpost patients to the correct healthcare professional.
  • To maintain accurate clinical records of all patient consultations and related work.
  • To support in the delivery of enhanced services and other service requirements on behalf of the PCN.

Sevenfields PCN – Nursing Associate

Full time – 37.5 hours

Salary – £21-23k Depending on Experience

We are looking for a Nursing Associate to join our growing multi-disciplinary workforce. These new roles within the primary care setting will provide an exciting opportunity for you to use your skills to support and complement the wider practices clinical team and provide patient centred care across the population.

We will consider applications from Trainee Nursing Associates.

Main Duties and Responsibilities include:

  • Assist at clinics, ensuring patient care by chaperoning and assisting patients to dress and undress.
  • Conduct weight and height measurements, test and record blood pressure and blood sugar levels, pulse rate and temperature.
  • Undertake simple dressing procedures following practice guidelines and using aseptic technique.
  • Assist with the provision of health checks.
  • Support immunisation programmes.
  • Provide healthy lifestyle advice regarding diet/exercise/smoking.
  • Ensure adequate infection control procedures in treatment and consulting rooms.
  • Communicate effectively with other members of the primary health care team and patients.
  • Preserve confidentiality.
  • Empathise with, and support people to improve and maintain their mental and physical health and wellbeing.
