We are delighted to report excellent results for Macmillan toolkit, Diabetes All 3 Treatment Targets and Serious Mental Illness Physical Health Checks completed by all general practice in Lewisham.

The Macmillan Toolkit for Cancer Care is a practical toolkit written by Macmillan GPs to support practitioners to improve cancer care in general practice. The toolkit contains suggestions of how to achieve high quality cancer care and explores a range of areas in providing high quality primary care to patients. The Toolkit is split into 4 modules with tasks for GP Practices to undertake and audit.
We have had 100% return on all the toolkits.
Our 3TT programme aims to improve the diabetes care pathway in Lewisham in line with the Five Year Forward View national ambitions for transformation in diabetes. It uses targeted interventions focused on 3TT delivered through practice and hub level to improve the achievement of the NICE recommended treatment targets (HbA1c, cholesterol and blood pressure) and driving down variation between GP Practices.
We achieved 49% smashing the 46% target with 8CP is at 48%.
