One Health Lewisham is the federation of all the General Practices within the London Borough of Lewisham, working collaboratively to ensure high quality, equitable, and sustainable primary care across Lewisham.

Established in 2017, One Health Lewisham is wholly owned by the GP surgeries of Lewisham, together representing over 300,000 patients. We work on behalf of practices, patients, and staff to deliver healthcare services in the community.

One Health Lewisham has grown out of four neighbourhood GP federations in the North, Centre, South-East, and South-West of the borough. Our work remains rooted in these local communities as we develop pan-Lewisham working to reduce variation and improve quality for patients, achieve economies of scale, and support practices.

Current OHL Vacancies

 

North Lewisham PCN Social Prescriber

Hours: 37.5 hours a week

Salary: £20-£24k p.a./pro rata based on experience

Working for North Lewisham PCN (Deptford/New Cross), you will support referrals from GP practices within the PCN, as well as self-referrals from multi-disciplinary teams and clients themselves. You will work closely with the PCN, attending meetings throughout the month. With the client, you will discuss their needs and identify a range of options that will better connect them with the local community and assist them to improve their wellbeing. You will co-produce a personalised support plan with the client and so good written skills are essential, as is an understanding of the barriers people face when accessing support. You will be able to manage and prioritise your own caseload with limited supervision and ensure that data is entered accurately and on time.

We welcome candidates from all backgrounds and with all qualifications. Essential experience: working with diverse communities, caseload management of varying degrees of client complexity, lone working. Must have outstanding interpersonal skills, whether with GPs, community organisations or clients. Should be comfortable carrying out home visits. Excellent written skills for client letters, case notes and short reports. Empathetic, positive disposition, excellent active listener.

Based at The Vale Medical Centre in Forest Hill, but with a large portion of time spent in the community (borough of Lewisham, with home visits around the Deptford/New Cross area).

Sevenfields PCN Community Link Worker

Hours: 37.5 hours a week

Salary: £23k p.a.

Do you care passionately about the health and well being of the people of Downham, Grove Park and Catford?  We are seeking an innovator and motivator to make a significant contribution to the transformation of Care locally ensuring that the PCN can both survive and thrive while  contributing to the delivery of new models of care that are self-sustaining financially and effective.

An innovative, entrepreneurial team builder is sought to lead on social prescription which empowers people to take control of their health and wellbeing. You will be able to spend time with GP referrals to focus on ‘what matters to me’ and take a holistic approach to their well being. Supporting people to link into community-based and community-led social, practical and emotional support, as well as statutory services. We aim to increase people’s active involvement in their local communities.

There are 3 major areas of work:

  • motivate and empower individuals to improve their health and wellbeing
  • strengthen GP’s, PCN, One Health etc. partnership working
  • build partnerships through collaborative working with non-statutory allied partners

The role of this link worker will be to draw together all partners in the PCN who together contribute to the health and wellbeing of the residents, co-ordinating a shared response, facilitating understanding between partners, developing an asset register and creating imaginative ways of asset sharing and promoting community engagement and empowerment across all ages of the population with especial emphasis on obesity, diabetes, active life styles and mental health interventions.

This role may involve working with specific grounds (i.e. children and young people) depending on the applicants experience. Sevenfields PCN Associate Physician 

Location: At a practice to be confirmed

Salary: Circa  £37.k (Negotiable based on experience)

Working Hours: 37.5 Hours – Part Time or Job Share considered

This is an exciting time to join one of the most progressive Primary Care Networks (PCN) in South London supporting over 60,000 patients and a portfolio of 70+ Care Homes or varying types and sizes. The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Leadership will be provided by the PCN’s Clinical Directors and One Health Lewisham our local GP Federation.

Your role will require you to participate in all aspects of patient care through knowledge gained from your PA training. You will be working as part of an Multi Disciplinary Team including a Care Coordinator, Care Planner, Clinical Pharmacist, Social Prescriber, Health Coach, and will be supported and supervised by a GP.

Sevenfields Primary Care Network (PCN) is one of the largest PCNs (of six) in Lewisham and is seeking to be a key driving force for improving community health and wellbeing in the neighbourhood. Our PCN is made up of 6 GP Practices serving a population of around 70,000 patients. We are led by a local GP who performs the role of Clinical Director: Dr Helen Tattersfield (Oakview Family Practice).

Our local population is somewhat older than other parts of Lewisham and contains several large care homes. It includes areas of extreme socioeconomic deprivation and the PCN is keen to find innovative ways of addressing the health challenges that accompany this. We are supported by One Health Lewisham (OHL) the GP Federation for Lewisham.

The post holder will have a key role in supporting our work in health promotion particularly in the work on diabetes prevention and control and will support the practices in this and other health promotion work.

The post holder will also work closely with allied community health and social care workers and will support service integration work around mental health, social care, community nursing and the voluntary sector.

The post holder will be employed by One Health Lewisham which is the host organisation that has been selected by Primary Care Networks in the borough to provide support and hold the funding for the networks

Clinical Lead for CEG Lewisham

Hours: 6 hours a week (2 sessions)

Responsible for:

Improving population health outcomes in Lewisham in line with the local post COVID-19 recovery plan and Clinical Effectiveness Group.

The Clinical Lead will also be responsible for providing clinical leadership within the Clinical Effectiveness Group, work closely with the data analyst, work strategically with input from key stakeholders and supervise the GP fellows.

Advanced Nurse Practitioner (Home Visiting)

One Health Lewisham (OHL) GP federation is recruiting to the role of Advanced Nurse Practitioner as part of a brand new general practice home visiting service in Lewisham. The role is available as a WTE (37.5hour per week) or on a part-time basis (20 hours per week minimum)

The Lewisham Home Visiting Service is an OHL developed service which will support local GP Practices by conducting rapid home visits by clinicians to housebound patients in order to deliver care in the community and help them to manage their demand.

One Health Lewisham is very excited to announce that we currently looking to recruit four dynamic, hard-working and ambitious GPs to work as Population Health Fellows alongside our local Clinical Effectiveness Group.

Your role would concentrate on improving population health outcomes in Lewisham in line with the local post COVID-19 recovery plan and clinical effectiveness group. Key areas of focus include HIV, latent TB, diabetes, obesity, hypertension, NHS health checks and preventative medicine.

We are currently aiming to recruit for an October 2020 start.

The posts will typically consist of 2 portfolio sessions* to focus on Population Health/Quality Improvement in the following streams:

· Migrant Health – including HIV and BAME/ethnicity related

· Digital health and education

· Lifestyle Medicine – including prevention and active treatment for diabetes, hypertension and obesity

· Mental Health, learning disabilities and social prescribing

The post holder will also be expected to undertake 2 clinical sessions/week in a primary care setting in Lewisham (GP,UCC etc).

Sydenham Green Group Practice – MEDICAL RECEPTIONIST

Working in the afternoon/early evening (2.00pm to 7/8pm) as well as occasional mornings (8am to 1.30pm) Monday to Friday (approximately 24 to 30 hours per week).  This will include Friday evenings until 7pm.

Flexibility in hours to cover holidays/sickness is essential

6 months will be a probationary period during which pay will be £9.76ph. On satisfactory completion of probationary period pay will be £10.76ph.
